Terms and conditions

These terms and conditions apply to every treatment agreement between Zonnelach dental practice (Gein 8, 3068 GJ Rotterdam) and its patients. By making an appointment you accept these terms.
1. Appointments
Appointments can be made by phone, email or via the website. Please arrive on time. Cancel or move appointments at least 48 hours in advance, otherwise we may charge the reserved time.
2. Rates and payment
We apply the rates set annually by the Dutch Healthcare Authority (NZa). Billing is handled by Infomedics. Payment is due within the term stated on the invoice. See our rates and insurance page.
3. Insurance
Reimbursement depends on your policy. You are responsible for knowing what your insurer covers and for paying the remaining balance.
4. Treatment plan and consent
For larger treatments we prepare a written treatment plan and cost estimate. Treatment only starts after your consent (WGBO).
5. Duty to inform
Inform us of medical conditions, medications and allergies. Report changes in address, insurance or health promptly.
6. Complaints
Unhappy with something? Talk to us first, we prefer to solve it together. If needed you can turn to the KNMT complaints scheme, of which we are a member.
7. Liability
Our liability is limited to the amount paid out by our professional liability insurance in the relevant case.
8. Applicable law
Dutch law applies to these terms and to the treatment agreement.
